This was a mildly painful lesion on the back of a 55-year-old woman. The presentation and histology are very typical. At low power one sees the "blue balls in the dermis", almost resembling a dermal lymph node. The basophilic cells with abundant interspersed basement membrane material are appreciated even at low power. Identifying involvement of an eccrine gland, as seen in this case, is unusual, but supports the notion of its eccrine origin.

This one focus at medium power demonstrates the lesion's involvement of an eccrine lobule.
