This healthy 6-month-old girl developed a low grade fever, incresed fussiness, and generalized erythema with scaling, crusting, and fissuring accentuated around the mouth and skin creases. An erosion appeared after her mother placed her on the examining table typical of a Nikolsky sign. Methicillin sensitive Staphylococcus aureus grew from a skin culture taken from her nares. She did well on oral antibiotics, acetominophen, and topical emollients with drying of the crusts and clearing of the erythema in 2 days and diffuse desquamation 5 days later.
