This 62 year old woman with longstanding bilateral venous stasis dermatitis developed progressive induration, fibrosis, atrophy, and hyperpigmentation of the left medial calf. Note the inverted chanpagne bottle appearance of the lower leg from fibrosis and atrophy of the subcutaneous fat of the medial calf. The large plaque was pruritic and painful and responded partially to topical steroids, elevation, compression stockings, and local care.
