A healthy 3 year old boy had a persistent diaper rash with bright red macerated erosive patches in the groin creases, around the anus and on the shaft and base of the penis. The lesions were painful and itchy and failed to respond to topical steroids, antifungal creams, antibiotics, and barrier ointments for 6 months. A culture for Group A beta-hemolytic streptoccus was positive, and the erosions healed after 10 days of oral amoxicillin. He was treated with oral antibiotics for 3 weeks.
