This healthy 6-year-old boy complained of a sore bottom and painful defecation for 3 months. A culture from the perianal skin grew Group A beta-hemolytic streptoccus. He was treated with oral amoxicillin for 3 weeks, and the weepy red patch resolved with the symptoms.
