A 13 year old complained of an itchy rash on his left hand and right foot and nail changes of the left thumb and right great toe. A potassium hydroxide preparation of scale from his left hand showed branching hyphae, and a culture from his right great toe nail grew Trichophyton rubrum. The skin rash and nail dystrophy cleared with oral itraconazole given for 5 months.
