This adolescent boy was misdiagnosed with alopecia areata and treated with oral antibiotics and topical steroids. Although the erythema, pustules and itching subsided the scale and subtle erythema became confluent on the back of the neck and scalp. He eventually cleared when tinea capitis was diagnosed and he was treated with an oral antifungal medication.
