A healthy 10 year old boy developed widespread scaly red itchy round patches of alopecia on his scalp. A potassium hydroxide examination of scale from his scalp showed endothrix infection with spores in hair fragments, and a culture grew Trichophyton tonsurans. The lesions improved markedly within 10 weeks, and a culture obtained at a 10 week check up was still negative 2 weeks later when the griseofulvin was stopped.
