This 25-year-old man with alopecia areata developed a diffuse nail dystrophy. Trachonychia, or rough nail, is characterized by excessive ridging or confluent pitting of the nails giving a sandpaper effect. It may be associated with alopecia areata, psoriasis,l and ichen planus but most commonly occurs as an isolated nail abnormality.
