This vellus hair cyst was an isolated lesion on a 34-year-old Asian male, and likely idiopathic. When multiple, they can be associated with eruptive vellus hair cysts, an autosomal dominant disorder. The chest and abdomen are common places for them. Histologically, they are distinctive, as they often have multiple vellus hairs within the cyst lumen, as in this case. Sometimes follicles are present in the cyst wall, supporting their follicular infundibulum derivation.

Description

Histologic sections of skin show a bland-appearing cyst deep within the reticular dermis. At low power, the lining appears squamous with abundant hairs within the lumen.
