This is an example of a myrmecia (ant hill-like) wart. This type of verruca palmoplantaris has striking viral cytopathic effect as described. These are Human Papillomavirus-related lesions, with this variety often involving HPV-1 and HPV-4. These lesions tend to evolve and not always show these obvious inclusions, rendering diagnosis a little more challenging.

Description

There is a papillomatous proliferation involving volar skin. The lesion forms a 'cup' shape, making an endo/exophytic lesion. The digitations have overlying mounds of parakeratosis. Inclusions are abundant. The papillary dermis shows fibrosis and tortuous vessels.
