This healthy 9-year-old boy was noted to have a white forelock at birth as well as light patches on his mid forehead, trunk, and legs. Within many of the hypopigmented and depigmented patches he had dark pigmented macules. He also had lateral displacement of the medial canthi, but his hearing was normal. His father and paternal grandfather had similar lesions.
